You do not need to have an application on file with the cemetery. You may choose to submit one in advance.
How to pre-register
Complete the application form and submit a copy of your discharge document:
- For active duty: DD Form 214 (or an equivalent document)
- For National Guard or Reserve: NGB Form 22
These documents must show that you meet eligibility requirements. The cemetery must verify eligibility before any interment can take place.
Final arrangements
Your funeral director will contact the cemetery to complete final arrangements.
Veterans with more than one period of service
If the veteran served more than once, submit discharge documents for all periods of service.
The cemetery uses these documents to prepare the headstone. The headstone may include:
- War service
- Highest rank achieved
- Significant decorations
- Branch(es) of service
